Insurance companies require a physician’s order to submit a claim for COVID testing. Please bring a copy of your insurance card and prescription for a COVID test with you to your appointment. If you do not wish to bill your insurance or obtain a doctors order our self-pay price is $150 and will be collected (credit card or cash) when you arrive at the lab.

When you arrive at the lab please wait in your car and call 833-265-8430. We will come out to the parking lot.
